Community Support


Assessment, planning and intervention are offered to cover all aspects of disability arising from acquired brain injury. Specific interventions are negotiated with funders upon referral, but services can be generally thought of in three principle categories:


Systematic Evaluation

  • Gathering and examination of existing medical, rehabilitation and neurological assessments,
  • Gathering of further third party information as required,
  • Provision of further assessment as indicated (recall analysis, prospective memory tests, task sequencing, etc.)
  • Delineation of physical, emotional, behavioural and cognitive issues in functional terms regarding the impact of disability and remaining abilities will have on ongoing capabilities,
  • Creation of a comprehensive rehabilitation plan to support recovery and compensation to the greatest degree possible,
  • Implementation and monitoring of interventions to ensure goal-orientation.

Community Living Support

Utilization of the Functional Evaluation data to determine appropriate community living to achieve the greatest degree of independence with an appropriate degree of safety. May include:

  • physical layout analysis
  • resource assessment
  • transportation review
  • daily living skills assessment
  • other aspects to successful community living.

The Community Support Coordinator remains available through all levels of plan implementation, working with the Support Facilitator to ensure the greatest success possible.

 

A significant component of our work is to ensure others involved with support plans are trained in the specific sequelae facing each individual. As strategies to counter cognitive and behavioural deficits are highly individualized, generalization of strategies is paramount to success.

 

Community Integration
Development of strategies to assist individuals to compensate for losses with a target at re-establishing each client back in their community of choice. This may include:

  • Self esteem counseling
  • Life skills training
  • Adjustment to relationship changes
  • Social skills training
  • Avocational interests
  • Maintenance of healthy lifestyle conducive to participation and playing a role within a community

The Society has developed numerous strategies dealing with emotional management and self esteem beneficial to overall community re-entry.

 

As with all interventions, coordination of rehabilitation and support services is critical to the overall success of the clients’ efforts.
